Payment policy
It is office policy to collect all cosmetic payments and medical copays/coinsurance before being seen by the practice. If you have a deductible, please be prepared to pay that at the end of your visit if applicable. No exceptions may be made, and failure to comply may result in having to cancel the appointment.

What to bring
- A photo ID and your current insurance card
- A list of your current medications and known allergies
- Any referral or records your insurer requires
- Notes on skin concerns, symptoms, or spots you want reviewed
